Regimental Number 5304 Montague Henry Faulkner, 13th London Regiment, Private, theatre of war 1(a) [I believe that is France] from August 20, 1916 to October 21, 1916, remarks D/W [I presume it means died of wounds] October 21, 1916.

The research of Ronald Leech, a relative, found that the 13th London Batallion participated in the Battle of Transloy Ridges from October 11 to October 19, 1916. Thus he was probably wounded in this action.
